Submit an incident for any and all incidents that involve or affect a foster child. If it is determined that an incident report should have been submitted and wasn't, a correction order will be issued.
At AIFACS we understand the importance of taking care of yourself and your family as you care for foster children. This can include using respite. Respite allows the foster family a much-needed break. We want to ensure that we are taking part in stopping/ slowing the spread of COVID for the family requesting respite as well as the family providing. It is important to remember that this is subject to change.
AIFACS uses the Minnesota Department of Health, Department of Human Services and the Centers for Disease Control for a resource and guide.
This form needs to be filled out before the respite takes place. If not, the respite provider will not be paid.
If respite is to take place, the foster parent requesting the respite is responsible for providing a mask for their foster child going into respite.
This form must be filled out by the FOSTER PARENT USING THE RESPITE. Once it is submitted, it will be given to your licensor in order for the respite provider to get paid. The respite provider will ONLY be paid if this form is filled out and ONLY if there are respite days available. If a foster parent(s) use respite but do not have any respite days left, the foster parent(s) will be responsible to pay the respite provider out of their own pocket at the rate of $50/child/night.
